Subject: Choir and Altar Server Annual Awards

Below you will find the 2003 application forms for the Gomidas and Yegmalian Awards to choirs and altar server programs. The awards will be presented at the 57th Annual Choir Assembly in Fair Lawn, NJ, on October 26, 2003.

The Central Council decided to eliminate the many lengthy questions which had appeared on previous "Choir Report Forms". Hence, this year's form is significantly shorter than last year's form. This has been done to encourage a response from more choirs throughout the Diocese. The deadline for submission of the form is noted below in the form.

Please note: The form mus be signed to be eligible for consideration for the awards.
